A vacation request form - or a leave request form for employees - is one of the most common HR forms used by employees for submitting requests for a long-term scheduled absence from work. These forms are then used by the managing personnel for reviewing, approving or denying these requests and updating the employee vacation calendar to avoid any shortages of manpower. Employee vacation request forms and time-off requests are distributed and collected by the HR or team leads and used later when tracking the total time off taken and calculating payroll.
Each request must feature the desired dates and days for the vacation. Ideally, the request should include a first and second choice for time off due to possible difficulties with varying shifts - especially with larger businesses.
Most states have laws that actually require employers to keep track of the time their employees spent off work. If your team always forgets how much time off they have accrued throughout the year, then having a record of all submitted vacation requests might make your business much more organized.
