A Time-Off Request Form is submitted by an employee to their higher-ups when they wish to request time off of work for a vacation, a sick day, an emergency or any other short-term need. These are usually paid but otherwise may vary depending on the policy of a particular business. If the employee wishes to ask for a scheduled absence from work to tend to their personal needs they may request a leave of absence, which is unpaid.
The Time-Off Request Form is used to formally document the employee’s request and the decision of the employer either approving or rejecting that request. On the side of the employer, these forms allow the management to keep track of the employee’s requested leaves in order to adjust work hours and avoid any possible shortages of manpower.
Your every request for time off must include the same key elements to make it effective. It as to specify the dates and the total days or hours of the requested leave regardless of whether you’re using a ready-made template or creating your own from scratch. Be sure to record the date when the request is being submitted (handy in case of any future disputes) and the name of the personnel manager or employee handling your paperwork. Managers will then have to follow up with an approval or denial date, a signature, and - preferably - the reason for denial.
