A massage intake form is a form used for documenting the health condition and pain management needs of a client before therapeutic massage sessions. When used with SOAP notes, intake forms help record and review the status of regular clients' progress before every appointment. They are important tools that help maintain and build a client base by creating a professional environment and are crucial when ruling out contraindications and dealing with insurance companies.
All massage therapy intake forms must include the massage patient’s informed consent and waiver that they sign to signify their consent to receive therapy at the clinic and waive and release their massage therapist from any and all past, present, and future liability, loss, cost, claim, or damage.
