What Is PS Form 3801?

PS Form 3801, Standing Delivery Order - also known as USPS Form 3801 - is a form used for authorizing an agent to receive restricted delivery mail on behalf of the addressed recipient, which may be an individual or a firm. PS Form 3801 Instructions

The Standing Delivery Order informs the USPS about the agents authorized by the addressee to receive the mail in their name. Filling out the form is quite simple. The addressee, which may be an individual or a firm, must complete their personal information at the top of the form and include a phone number where they can be reached. Every new USPS 3801 Form will supersede any previous form submitted to the USPS. The addressee can authorize more than one agent, and the services included are the following:

  • Adult Signature Required;
  • Certified;
  • Insured;
  • Collect on Delivery (C.O.D.);
  • Priority Mail Express;
  • Signature Confirmation;
  • Unrestricted Registered Mail.

In order to receive the article, the agent has to sign the form. It is important to highlight that this form includes the Adult Signature Restricted Delivery option, which is a USPS service that includes verification of the age of the recipient as a condition for accepting delivery. A valid ID must serve as proof of the age of majority. If the recipient is younger than 21, the mailpiece cannot be left at the address and will be returned to sender.

The Adult Signature Restricted Delivery fees are paid per piece and separate from postage and other fees. Said fees may be refunded by USPS only if it fails to provide the service. This service also provides electronic confirmation of the mail deliv­ery or attempted delivery. In addition, the recipient's signature confirmation can also be provided upon request. The USPS keeps a record of delivery with the recipient's signature for a determined period of time. The following are two ways in which a recipient can get a confirmation of delivery information for an adult signature:

  • Through the USPS website;
  • By calling 800-222-1811.

In both cases, the addressee must provide an article number. Upon request, they will receive a letter containing evidence of delivery either via fax, e-mail, or mail.

Shaded areas of the form are not to be filled out by the addressee, but by the USPS employee instead. The form includes Customer Instructions, which guide the addressee through the completion of the Authorized Agent grid.
