What Is DR 1094 Form?

Form DR 1094, Colorado W-2 Wage Withholding Tax Return, is used by Colorado employers to report Colorado W-2 income taxes that have been withheld from employee pay. For more detailed information regarding filing frequencies and requirements, refer to publication FYI Withholding 5, Colorado Wage Withholding Tax Requirements. If the employer did not withhold any taxes during the filing period, a 'zero return' is to be filed to avoid being billed by the Department. A paper zero return may be mailed or a return can be filed electronically by accessing the Colorado government website. You can not file a paper DR 1094 if you remitted your withholding taxes by EFT. Employers who pay more than $50,000 in withholding tax per year are required to pay by EFT.

How to Fill Out Form DR 1094?

Follow these DE 1094 Form instructions below.

  1. Enter the Colorado Business Account number (8 digits) from your withholding certificate. Enter the FEIN number for your organization, Social Security number (if applicable) you provided on your application, the filing period for this return (be sure to use the correct period end date for your determined filing frequency), and your Colorado account mailing address.
  2. Line 1. Enter the amount of Colorado income tax withheld for the period.
  3. Line 2. Enter any overpayment amount if a previous period in the current tax year was overstated and paid. To calculate this overpayment amount, complete the worksheet (first page of full form DR 1094). If additional tax is owed, file another return for the period the tax is due, reporting only the additional amount owed.
  4. Line 3. Subtract Line 2 from Line 1.
  5. Complete Line 4 only if the return is being filed after the due date. The penalty is calculated by determining how long past the due date the return is being filed.
  6. Complete Line 5 also only if the return is being filed after the due date. Refer to publication FYI General 11, Colorado Civil Tax Penalties and Interest.
  7. Line 6. To calculate the amount due, add together the amounts entered on Lines 3, 4, and 5.
