Form R-229, Application for a Non-commercial Learner Permit and/or Driver License, is a formal document signed by Connecticut residents who want to receive a learner permit or driver's license. Form R-229 Instructions

Follow these steps to fill out the R-229 Form:

  1. Indicate your full name, sex, date of birth, height, eye color, mailing, and physical addresses, and telephone number. Add your social security number and list other names you have used before.
  2. State whether you are a U.S. citizen and Connecticut resident.
  3. Confirm your wish to participate in the organ and tissue donor program.
  4. Answer several questions about your driver's license or permit history - check the boxes to say "yes" or "no." You will have to tell the authorities that you have failed a driver's license examination before or one of the documents you are applying for now has been issued to you in the past. Confirm whether your license has even been suspended in Connecticut or in a different state.
  5. Certify you do not have any medical problems that would negatively affect your driving skills.
  6. Confirm all your statements are true and correct, sign, and date the document.

The rest of the application will be filled out by the DMV representative once you visit a local DMV office to present the required documentation. You also need to submit these documents:

  • Identification document and proof of Connecticut residency (two documents or envelopes that were sent to you to your residence address within ninety days). You also need to show a document that proves your social security number - it can be either a social security card or a tax form issued within the last five years;
  • Certificate of Parental Consent if you are a minor that applies for a learner permit;
  • Proof of paid application fees - $40. You may pay at the DMV branch office using a convenient method of payment - a credit card, money order, check, or cash.

After your visit, you may track the delivery of the license on the DMV website - it will be sent to the mailing address indicated in the form.
