The Form 1040-V due date for taxpayers in most states is April 15. In Maine and Massachusetts, the deadline is extended to April 17 due to state holidays. General Form 1040-V instructions are attached to the form. Read them carefully before completion. Detach the filled out form before mailing it.

Use the following instructions to prepare your IRS payment voucher:

  1. Pay electronically (online, by phone or another mobile device) to avoid delays;
  2. When filling voucher for a joint return, enter the SSN provided first on the return form in Box 1, and the second SSN in Box 2 correspondingly;
  3. Enter all names and address exactly as provided on a tax return form;
  4. Print all the information clearly;
  5. If you choose to pay using the IRS website, do not fill out this form;
  6. Do not enclose any cash to your voucher. To pay by cash, you need to register online at the address provided in the instructions;
  7. Do not send a single check for amounts of $100,000,000 or more. If you want to send $100 million or more this way, spread the payments over several checks;
  8. Place the IRS Form 1040-V loose in your envelope. Do not staple or attach it in some other way to your tax return form.

Where to Mail Form 1040-V Payment Voucher?

The taxpayers from different states are required to use a different address to mail 1040-V:

  • If you live in Florida, Louisiana, Mississippi, or Texas, send your payment voucher to Internal Revenue Service, P.O. Box 1214, Charlotte, NC 28201-1214;
  • Taxpayers from Alaska, Arizona, California, Colorado, Hawaii, Idaho, Nevada, New Mexico, Oregon, Utah, Washington, and Wyoming are required to mail to Internal Revenue Service, P.O. Box 7704, San Francisco, CA 94120-7704;
  • If you are from Arkansas, Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota, Montana, Nebraska, North Dakota, Ohio, Oklahoma, South Dakota, or Wisconsin, send the IRS Form 1040-V to Internal Revenue Service, P.O. Box 802501, Cincinnati, OH 45280-2501;
  • The residents of Alabama, Georgia, Kentucky, New Jersey, North Carolina, South Carolina, Tennessee, and Virginia must submit their payments and vouchers to Internal Revenue Service, P.O. Box 931000, Louisville, KY 40293-1000;
  • If you live in Delaware, Maine, Massachusetts, Missouri, New Hampshire, New York, or Vermont, mail the form to Internal Revenue Service, P.O. Box 37008, Hartford, CT 06176-7008;
  • For Connecticut, District of Columbia, Maryland, Pennsylvania, Rhode Island, and West Virginia, the mailing address is Internal Revenue Service, P.O. Box 37910, Hartford, CT 06176-7910; and
  • A foreign country, American Samoa, or Puerto Rico residents, as well as dual-status aliens and nonpermanent residents of the United States Virgin Islands or Guam, must mail the IRS payment voucher to Internal Revenue Service, P.O. Box 1303, Charlotte, NC 28201-1303.

How to Pay IRS 1040-V Online?

IRS 1040-V Form contains a link you can use to pay your taxes online. The online payments can be made in two ways: through the direct payment service and through the IRS-approved payment processor. To choose the first option, follow the provided link, and click "Direct Pay." Use it to pay your Form 1040 series taxes directly from your checking or savings account.
