What Is DD Form 518?

DD Form 518, Accident - Identification Card, is a form filed by U.S. Army service members involved in any sort of traffic accident while driving government-owned vehicles. The service member must fill out a DD 518 in order to provide the other party involved in the mishap with their personal identifying information.

The latest version of the form - sometimes incorrectly referred to as the DA Form 518 - was released by the Department of Defense (DoD) on October 1, 1978 . A copy of the DoD Accident Identification Card is placed in each vehicle for the purposes provided above. When an Army vehicle is involved in a mishap, the driver provides the injured party with a properly filled-out form. All soldiers operating government-owned vehicles are responsible for reporting all accidents to military and local Law Enforcement, their immediate supervisor, and Vehicle Control Officer (VCO) for further investigation.

DD Form 518 Instructions

A service member involved in a car accident has to fill out the DD Form 518 at the scene of the mishap and give a copy to the drivers of all other vehicles involved. If the crash involved a parked car with the owner of the vehicle absent from the scene, the DD Form 518 should be placed in or on the parked vehicle. The police should be notified immediately. The service member must remain by the other car until either the police or the owner of the car arrive at the scene of the accident.

When an Army-owned vehicle is involved in an accident or a car crash, Unit Commanders must decide whether the service member or the unit will provide the funds for the repairs. Normally, the unit provides funding unless the individual has been proven to be guilty of causing the mishap.

The commander will make the decision if the service member needs to have their Government Motor Vehicle License revoked. These licenses can only be reinstated after remedial training with a certified vehicle trainer.

  1. In cases of on-base vehicle accidents service members should stick to the following procedure:

    • Call the Security Forces (Law Enforcement Desk, 882-2502/2503);
    • Call 919 SOW Safety;
    • Call the supervisor;
    • Call the Vehicle Control Officer (883-6106);
    • Complete the Standard Form 91, a copy of which must be located inside the government vehicle;
    • Complete the DD Form 518 (if another vehicle is involved).
  2. In case of OFF base government vehicle accident a person should stick to the following procedure:

    • Call the Local Police/Ambulance (911), Ambulance call is necessary only in case it is required by injury;
    • Call 919 SOW Safety (883-6723/6724);
    • Call the supervisor;
    • Call the Vehicle Control Officer (883-6106);
    • Complete the Standard Form 91, which is located in the government vehicle;
    • Complete the DD Form 518 (if another vehicle is involved) and give it to that person.

Individuals serving in the Air Force will also turn inAF Form 2293, United States (U.S.) Air Force Motor Vehicle Operator Identification Card, to the wing VCO.

How To Fill Out the DD Form 518?

  1. Block 1 is for providing a personal address for any correspondence regarding the accident.
  2. The date of the accident must be entered in Block 2.
  3. The make and type of vehicle should be specified in Block 3.
  4. The next item - Block 4 - is for the Registration Number of the car.
  5. The driver's name - their first, last, and middle initials - must be provided in Block 5 (DRIVER).
  6. Block 6 is for specifying the SSN and the Grade of the soldier.
  7. Block 7 is for specifying the soldier's organization.
