How to Fill Out Form CS01?

You will need to include the following information to complete a Form CS01:

1. Your organization's number and full name.
2. The confirmation date for your organization (the date it was founded). If you are unsure of this date you are able to check online through the Companies House's website and update the confirmation date if necessary.
3. The signature of the person completing Form CS01. Note, only a high-level official, such as a director or secretary should be completing this document. If you are unsure if your role at your organization makes you eligible to complete the form you can check section 270 or 274 within the 2006 Companies Act.
4. Information for the person completing this document. Include the company information such as name, address, and phone number. Note, any information listed on this form will be made available for public record.

Review the checklist to confirm all necessary information has been included and double-checked against the information currently listed with Companies House. A forty-pound fee must also be included at the time of filing Form CS01 and payments made by cheque must be payable to "Companies House."

If your company or organization is located outside of England, you have the option to send your form to additional addresses located in Scotland, Wales, or Northern Ireland that are listed at the bottom of the form. This form can also be filed online through the Companies House's website.
