INSTRUCTIONS ON FILLING OUT FORM P-2
All pesticide products and non-chemical pest control devices distributed or sold in the State of Hawaii must be licensed under
the provision of the Hawaii Pesticides Law (Chapter 149A, Hawaii Revised Statutes) and the Administrative Rules, Chapter
66, Pesticides, Title 4, Department of Agriculture. The license expires on December 31 of each third year and must be
renewed thereafter as long as the product is being distributed or sold in the State of Hawaii.
A. Submit one application form for each product being licensed. Complete the form as follows:
Lines 1, 2, and 3: Self-explanatory.
4. Show name and address of principal Hawaii distributor of the product. If more than one, list on separate
sheet. If none, please so indicate.
5. Show brand name, EPA registration number, and EPA establishment number. Check the applicable items.
Lines 6 and 7. Check the applicable types.
8. Show the net contents and type of container in which the product is to be marketed. If more than one
package size is used, list each one.
9. Authorized representative must sign and date the form.
B. Labels and Labeling
1. Submit one copy of the final printed label (Text.pdf, searchable and unsecured form only), the EPA
Stamped "ACCEPTED" label (with comment letter, if applicable) and all other labeling, brochures,
bulletins, and pamphlets that are required to accompany the product when sold or distributed.
2. For products with labeling stenciled or silk-screened on the container, submit a facsimile of the
label rather than the container. The facsimile must be a text-searchable, unsecured PDF file.
C. Other Required Information
1. One copy of the EPA Confidential Statement of Formula (EPA Form No. 8570-4) for the basic product
and for any EPA registered product that might be used in its formulation. Information on inert ingredients
will be kept confidential.
2. Requests to license sub-registered products must be accompanied by a copy of the EPA "Notice of
Supplemental Distribution of a Registered Pesticide Product" form (EPA Form No. 8570-5).
3. Submissions involving alternate product or brand names must be accompanied by one copy of the EPA
"Notice of Pesticide Amendment" (EPA Form No. 8570-1).
4. For products containing active ingredient(s) never before licensed in Hawaii, additional data will be
required to conduct a groundwater evaluation.
5. Products containing microorganisms must submit the HDOA, Plant Quarantine permit with their
registration packet. More information can be found here:

6. For non-chemical pest control devices, submit supporting information consisting of efficacy, safety,
and reliability data. See section F for more details.
D. License Fees
1. All pesticides:
$930.00 per product per three (3) year license period.
2. Non-chemical pest control devices: $330 per device; one-time fee

E. Restricted Use Pesticides
1. Any pesticide classified as "RESTRICTED USE" by the State and/or EPA may be sold only to certified
applicators by qualified dealers holding a RUP dealers permit. An out-of-state dealer who sells directly to
the end user in Hawaii must obtain a RUP dealer permit from this office.

F. Non-chemical Pest Control Devices: Supporting documents
1. Submit a description of the principles fundamental to the efficacy of the nonchemical pest control device,
a description of the tests conducted according to the procedures described below and test results
thereof on which claims are based, including efficacy, reliability, safety, and other supporting data that
shows the nonchemical pest control device will perform its intended function without causing
unreasonable adverse effects on humans or the environment.
2. Each test submitted shall be based on a written protocol that clearly indicates the objectives and all the
methods for the conduct of the test. The protocol shall contain, but not be limited to, the following
information:
a. A descriptive title and statement of the purpose of the study
b. The name and address of the sponsor and address of the testing facility at which the study was
conducted
c. Justification for the selection of the test organism
d. Where applicable, the number, body weight range, sex, source of supply, species, strain, sub-
train and age of the organisms tested
e. A description of the experimental design, including methods for the control of bias
f.
Where applicable, a description or identification of the diet for the test animals or fertilization and
irrigation schedules for plants used in the test
g. Treatments, such as the test frequency and volume for nonchemical pest control devices, and
the method and frequency of administration
h. The type and frequency of data collection, and measurements to be made
i.
The records to be maintained
j.
The date of approval by the sponsor and the signature of the test director
k. A statement of the proposed statistical analyses to be used.
3. Upon request from the Hawaii Department of Agriculture, the applicant shall provide any nonchemical
pest control devices in order to test nonchemical pest control devices to determine the reliability, efficacy
and safety of the nonchemical pest control device. If this occurs, the department shall provide the
applicant with a description of the tests to be conducted, and estimated schedule to complete the tests.
