Ppd Form Templates

The Ppd Form, also known as the Mantoux Tuberculosis (TB) Test Form or Tuberculin Skin Test (TST) Form, is used to assess an individual's risk for tuberculosis infection. The form is used to record the administration of the Ppd (purified protein derivative) test, which is the standard method for screening for latent tuberculosis infection. The Ppd test involves injecting a small amount of fluid, containing a substance derived from the TB bacteria, under the skin of the forearm. After 48 to 72 hours, the healthcare provider examines the injection site for any skin reaction, which helps determine if the individual has been exposed to TB bacteria. The Ppd Form is used to document the test results and other relevant information, such as the individual's medical history and risk factors for TB.
