DD Form 2906 Instructions
The employee (or the responsible supervisor) should fill in their name, DoD ID number, and the year the evaluation takes place in the top line on the cover page of the form.
Part A is for recording administrative data. The appraisal period - or appraisal cycle - is specified in Blocks 1a and 1b. The effective date of the rating of record is specified in Block 1c.
The name of the employee and their DoD ID number are once again specified in Blocks 2 and 3. Block 4 contains information about the employee's current position and its description code.
The current pay plan, occupational code, grade, and step are described in Block 5. Blocks 6 and 7 are for the name of the employee's organization and their duty station.
The employee, official, or higher-level reviewer certify their acknowledgment of performance discussions in Part B.
Part C - DoD Core Values and Organizational Goals, is completed by rating official and discussed with the employee during counseling.
Part D - Performance Element and Standards - is completed for every element. Multiple copies of the page are created for every element evaluated.
Part E - Performance Summary - consists of three columns. The title of each element identified in Part D is listed in Column B. Element ratings are specified in Column C.
A summary rating is calculated and specified in Box A1 of Column C. That summary rating is compared to the chart provided at the bottom of the page and the result is entered in Box A2.
The rated employee receives copies of all pages. The official will keep the original for records.
