What Is DD Form 2861?

DD Form 2861, Cross-Reference, is a form that is used for tracking down and verifying a cross-reference check of documents. When first introduced, the DD 2861 replaced Optional Form 21, Cross Reference Sheet .

The form was last released by the Department of Defense (DoD) on June 1, 2003 , with previous editions being obsolete. An updated fillable DD Form 2861 - sometimes incorrectly referred to as the DA Form 2861 - is available for download and digital filing down below or can be found on the Executive Services Directorate website.

DD Form 2861 Instructions

  1. Each page of the DD 2861 consists of three sections. Section I of the form is called the cross-reference section. This part requires name, number or subject under which the DD Form 2861 is filed.

  2. Section II, Identification, consists of four blocks.

    • Block A is for submitting the date of the document, which can also serve as a filing date of the form;
    • Blocks B and C are for submitting the 'TO' and 'FROM' addresses respectively. Organizational titles or locations can be used to identify to whom or from whom the letter is sent;
    • Block D is for a brief summary of the contents. Understandable and legible abbreviations must be used when summarizing the contents of the documents.
  3. Section III of the form - Filed - provides the name, number, or subject under which the document itself is filed.

The cross-reference sheets must be prepared in two cases. These are:

  1. When a document contains more than one name, number or subject under which it should be filed;
  2. When there are not enough copies of the record itself that are available for this purpose.

There are three types of cross-reference with a different amount of pages required for each case.

  1. For a single cross-reference, only the first page from the form is required to be filed. A cross-reference caption must be entered in open space opposite the right-facing arrow;
  2. For a double cross-reference, the first two pages from the form are filed. The first caption must be entered in the open space opposite the right-facing arrow and the second one in the shaded space that matches the open space on the second sheet;
  3. A triple cross-reference requires all three pages to be filed with captions in all three spaces provided.
