This form must be submitted by anyone requesting a search warrant record (except as provided in

Criminal Rule 37(b)
), including parties and attorneys. Court clerks are required to exclude certain

information protected by the Victims’ Rights Act (VRA)
when the records are requested by someone
other than defense counsel or the prosecuting attorney.
Access to search warrant records is governed by Criminal Rule 37(e) and Administrative Bulletin 86.
Administrative Bulletin 53 explains the VRA confidentiality requirements.
