SHIPMENT OF HOST PLANTS OF OAK WILT DISEASE FROM THE STATE OF
TO FLORIDA
IN COMPLIANCE WITH CHAPTER 5B-26 (OAK WILT DISEASE), FLORIDA ADMINISTRATIVE CODE (F.A.C.).
Permission is granted to the
____________________to authorize
firms
to ship host plants (oak (Quercus spp.), chestnut (Castanea spp.), chinquapin (Castanea spp. or Castanopsis spp.) and

, to Florida subject to the conditions of this permit.
CONDITIONS
1. Only firms that have signed a compliance agreement with the
______________________
will be authorized to ship host plants under this permit.
2. All host plants shipped into Florida under the master permit must have originated from a nursery or
production area:
a) Certified free of the oak wilt disease defined in Rule 5B-26.002 F.A.C.; and
b) Not located in or within 10 miles of any known infestation of the oak wilt disease defined in Rule 5B-
26.002, F.A.C.
3. The
________________ shall provide to the Florida Department of Agriculture and Consumer
Services, Division of Plant Industry, a list of firms authorized to ship under this master permit and their
identification number.
4. All shipments of host plants shall be stamped with or accompanied by a certificate indicating compliance with
Florida’s oak wilt disease rule, Chapter 5B-26, F.A.C. and authorization to ship under this master permit as
evidenced by their identification number.
5. Any shipment of host plants found to be infested with the oak wilt disease as defined in Rule 5B-26.002
F.A.C. shall be disposed of in accordance with Rule 5B-26.007, F.A.C. and acceptance of further shipments
of host plants from the party in violation shall be suspended. The suspension shall remain in effect until the
department of agriculture of the state of origin has notified the Division of Plant Industry in writing that the
problem has been resolved.
