When applying for registration for Full Fee power units over 60,000 pounds combined gross vehicle weight, you must
show actual Idaho mileage for the previous July 1 through June 30 mileage period, unless no Idaho miles were
traveled during that period. If no Idaho miles were traveled, mileage may be estimated based on the options below.
Instructions for Completing this Form
Fill in your account number and the registration year below. Then, select the estimated mileage option of your choice
for each unit. Write the mileage estimate for each unit on the Full Fee Application for Registration. Include this
signed form with your application.

You may use the standard mileage estimate of 11,000, which is the average annual mileage operated
per Full Fee vehicle in Idaho.
