Instructions for Form RC-10 Schedule Cd - out-Of-State Cigarette and Little Cigar Sales or Shipments

Schedule CD Instructions
General Information
• You must complete a separate schedule for each state into which you shipped or sold cigarettes or little cigars.
• Do not include any cigarettes or little cigars with Illinois tax stamps affixed that you
returned to manufacturers; and/or
sold to Illinois licensed out-of-state distributors.
• Attach a completed schedule to either Form RC-6, Cigarette and Little Cigar Revenue Return, and keep a copy for your
records.
Specific Instructions
Column 1:
Date you shipped cigarettes or little cigars out of state, include the month, day and year.
Column 2:
How the cigarettes or little cigars were shipped, if by
– distributor truck, enter DT
– common carrier, enter CC
– parcel post, enter PP
– customer truck, enter CT.
Column 3:
Invoice number for the cigarettes or little cigars you shipped into another state.
Column 4:
The Federal Employers’ Identification number (FEIN) of the person or company listed in Column 5.
Column 5:
Name and address (including the city) of the company or person to whom you shipped or sold cigarettes or
little cigars.
Column 6:
Number of packages containing 20 sticks that you shipped or sold.
Column 7:
Number of packages containing 25 sticks that you shipped or sold.
Column 8:
Number of sticks in packages containing cigarettes or little cigars in quantities other than 20 or 25 that you
shipped or sold.
Column 9:
Total number of sticks per invoice.
Column 10:
Indicate whether tax was paid to the state into which you shipped or transferred the cigarettes or little cigars.
