NOTE: Form instructions should not be included when the form is submitted.
This form shall be used when a Local Public Agency (LPA) is refunding or refinancing bonds issued to retire general improvement bonds or public benefits. It should be completed
with the assistance of the District MFT documentation reviewer and submitted to the appropriate district as soon as the bonds are sold. Please refer to the Local Roads and Streets
(BLRS) Manual, Chapter 15 for more detailed information. For signature requirements, refer to Chapter 2, Section 3.05(b) of the BLRS Manual.
Local Public Agency
Insert the name of the LPA.
County
Insert the name of the county in which the LPA is located.
Documentation Review Date
Insert the date of the documentation review.
Location and Description of Improvement
Name of Streets/Roadways
Insert the name of the street/roadways to be improved. This information can be obtained from the bond ordinance or from
BLR 15310 form. For additional locations use the add button.
Intersecting Streets
From
Insert the name of the intersecting street/roadway at the starting point. This information can be obtained from the bond
ordinance.
To
Insert the name of the intersecting street/roadway at the ending point. This information can be obtained from the bond
ordinance.
Construction Dates

Class of Bonds
Insert the class of bonds retired by refunding issues (e.g. MFT, General Improvement Bond, etc.).

Insert the date of sale of the original bonds.
Bond Numbers
Insert the bond number(s) of the original bonds refunded. If a number was assigned, it can be found in the bond ordinance
(e.g. Series 2007B).
Amount of Bond
Insert the dollar value amount of the original bond issue. This information can be obtained from BLR 15310 for the original
bond or from LPA's files.
Principal of Bond Retired
Insert the dollar value amount of the original bonds that have been retired to date. This information can be obtained from bank
records, the original bond issue principal and interest payment schedule and/or from BLR 153111 for the original bond issue.
Principal Outstanding
No entry required, this is a calculated field. It takes the principal of the original bond issue minus the principal of the original
bonds retired.
Amount of Interest
Insert the dollar value amount of the interest due on the original bonds at the date of refunding. This information can be
obtained from bank records, the original bond issue principal and interest payment schedule and/or from BLR 15311 for the
original bond issue.
Interest covered
Insert the dollar value amount of the interest of the original bond issue that will be covered by this refunding issue.
Other Costs
Insert the dollar value amount of other costs to be covered by this refunding bond issue (e.g., legal fees).

No entry required, this is a calculate field. It is the principal outstanding of the original bonds plus the amount of interest
covered by this refunding issue plus other costs covered by this refunding issue.
Date of Ordinance
Insert the date of the ordinance authorizing these refunding bonds.
Ordinance Number
Insert the ordinance number authorizing refunding bonds.
Date Sold
Insert the date the refunding bonds were sold.
Total Bonds Sold
Insert the total dollar value of the refunding bonds sold.
Premium/Discount
Insert the dollar amount of the premium/discount of the bonds sold.
