Instructions For BLR 15313
NOTE: Form instructions should not be included when the form is submitted.
This form shall be used to show the disposition of proceeds of a particular refunding/refinancing bond issue. It shall be completed as soon as bonds for refunding or refinancing
bond issues have been sold. This form together with form BLR 15312 should be completed by the clerk with assistance from the documentation reviewer, if needed and submitted
to the appropriate District. Please refer to the Bureau of Local Roads and Street (BLRS) Manual, Chapter 15 for more detailed information.
For signature requirements refer to Chapter 2, Section 3.05(b) of the BLRS Manual.
Local Public Agency
Insert the name of the Local Public Agency (LPA).
County
Insert the name of the county in which the LPA is located.
Section Number
Insert the section number assigned to the refunding.
Documentation Review Date
Insert the documentation review report date.
