What Is NAVPERS Form 1616/26?

NAVPERS Form 1616/26, Evaluation Report & Counseling Record (E1-E6) , is a formal document prepared to assess the performance of Navy enlisted personnel with the purpose of providing feedback to evaluated individuals and to assist with their improvement. This report contains information on members in pay grades E-1 through E-6 - apprentices and non-commissioned officers. The report must identify the strengths and weaknesses of each member, motivate them to do better in the future, and recommend them for promotion if necessary.

NAVPERS 1616/26 Instructions

Here is how you must complete the Navy Evaluation Form:

  1. Indicate the full name of the evaluated member, their grade or rate, and social security number. State the officer designator and the competitive status of the member.
  2. State the identification code of the member's ship or station and the name of the activity they have been assigned to.
  3. Enter the member's promotion status.
  4. Specify the date of the report, the reason for it, and the reporting period.
  5. Indicate the report type and write down the physical fitness assessment code.
  6. Write down the name, grade, designation, title, unit identification code, and social security number of the reporting senior.
  7. List the awards, achievements, and duties of the member during the reporting period.
  8. Enter the date of counseling, the name of the counselor, and obtain their signature.
  9. Review the performance traits of the individual in question and write down your marks in each field. You will need to assess the professional knowledge of the member, their quality of work, teamwork and leadership skills, character and adherence to Navy values, and personal responsibility for their duties. For each characteristic, you can write an X in the appropriate box. The report contains short descriptions of each grade to determine the necessary evaluation.
  10. State the individual trait average for the report.
  11. If you decide to recommend the member for promotion, you may enter the assignment or program they are qualified for. Alternatively, write down "None" if you think promotion is not possible.
  12. Write down your full name and rate. Date the form.
  13. If you gave marks below 3.0 for the member's quantity of work, you must provide comments in the available space.
  14. List courses, certificates, and qualifications attained by the member during the reporting period.
  15. Check the appropriate boxes to recommend promotion and/or reenlistment.
  16. Sign the document - it must be signed by the senior rater and the reporting officer. Obtain the signature of the evaluated individual.

Once the document is completed, it must be mailed to the Navy Personnel Command by the reporting senior - a commanding officer or an officer in charge - within fifteen days for active-duty members and thirty days for inactive duty members.
