What Is Form IMM 5444?

Form IMM 5444, Application for a Permanent Resident Card , is a form that individuals can use when they want to receive a Canadian Permanent Resident (PR) Card. The purpose of the document is to help individuals to receive their PR Card if their PR Card has expired, is lost or damaged, are not in their position within 180 days after they immigrated to Canada, or if they have changed their gender designation or name and would like to update that information.

Form IMM 5444 can also be used by those individuals who are applying for the PR Card for the first time. After the individual has filled out the application it is supposed to be mailed to the Case Processing Center, the address will depend on the delivery method and can be found on the official website of the Government of Canada.

How to File Form IMM 5444?

  1. Introduction. State your preferred language for correspondence (English or French), the date when you were granted PR status, and the purpose of your application.
  2. Personal Details. Use this section of the document to write down your full name, gender, date of birth, place of birth, citizenship, current country of residence, and other personal information. Provide information about your appearance, current residential address in Canada, and contact information.
  3. Immigration History. Indicate the place where and how you were granted PR status.
  4. Personal History. List the places where you've lived (both inside and outside of Canada), provide an overview of your education, travel, and work history.
  5. Residency Obligation. Complete this part of Form IMM 5444 if you have stayed outside of Canada for 1,095 days or more in the last five years.
  6. Consent to Disclose Information. Sign this part of the form to allow the Canadian Border Services Agency to share information about your travel history.
  7. Statement to Consent. Sign this part of the form to allow the IRCC to collect use your Social Insurance Number to collect information about your income tax.
  8. Declaration of Applicant. Sign this part of the form to certify that the information you have presented in the document is true and valid. Any false statement you have made can be grounds for potential prosecution.
  9. Solemn Declaration Concerning Lost, Stolen, Destroyed, or Never Received PR Card. If filing Form IMM 5444 due to the reasons stated in the title of this paragraph, you are supposed to provide information about your previous PR Card and describe the efforts you have made to find your card.
