The above-named young adult is no longer under the permanent custody,
foster custody, voluntary foster custody, or court-ordered temporary foster custody of
the DHS, and is no longer under the court’s jurisdiction pursuant to Hawai`i Revised
Statutes (“HRS”) Chapter 587A, such jurisdiction having terminated on ____________.
2.
The above-named young adult voluntarily consents to participate in the
Young Adult Voluntary Foster Care Program, pursuant to HRS Chapter 346 Part XIX;
3.
The above-named young adult has signed a Voluntary Care Agreement,
which is attached as Exhibit "A";
4.
The DHS has made reasonable efforts to achieve permanency for the
above-named young adult;
5.
It is in the best interest of the above-named young adult to extend foster
care services; and
6.
The above-named young adult was (check one):

Under the permanent or foster custody of the DHS at the time the
young adult attained the age of eighteen;
[ ]
A child who was placed in guardianship after attaining the age of
sixteen;
[ ]
A child who was adopted after attaining the age of sixteen: and
The above-named young adult is (check all that apply):
[ ]
Completing secondary education or a program leading to an
equivalent credential;
[ ]
Enrolled in an institution that provides post-secondary or vocational
education;

Participating in a program or activity designed to promote or
remove barriers to employment;
[ ]
Employed for at least eighty hours per month;
[ ]
Incapable of doing any of the activities described in Hawai`i
Revised Statutes §346-395(4)(A) to (D) due to a medical condition.
WHEREFORE, it is respectfully requested that (1) the Court grant this
Petition, (2) the Court assume jurisdiction of this matter, and (3) the above-named
young adult be permitted to enter the Young Adult Voluntary Foster Care Program
pursuant to HRS Chapter 346 Part XIX.
