How to Get a State Pension Statement?

You can complete the fillable application directly in your browser or print the form first and use a black pen to fill it out. There are other ways to obtain a forecast - you can request it online (on the Department for Work and Pensions website) or call one of the following numbers:

  1. If you are residing in the United Kingdom, call 0800 731 0175.

How to Fill Out Form BR19?

  1. Provide the full name, date of birth, address, telephone number, and National Insurance Number of the applicant requesting a personal pension forecast. Individuals born after 1961 can skip questions 10 through 12 and go straight to Part 2 of the application.
  2. Record the applicant's marital status and partner's date of birth and gender (if applicable).
  3. If the form is being filed by an authorized representative of the applicant, their full name, title, name of the organization, and address must be included on the form. Authorization must be confirmed with the Department for Work and Pensions in advance. The representative must be given permission to request and receive the applicant's personal forecast. Every forecast will need its own separate authorization.
  4. Applicants can ask the Department for Work and Pensions to send them the forecast in an alternative format - individuals may request a larger print or an audio version of the forecast. Additionally, the forecast can be provided in Welsh.
