- Information About the Individual. Applicants should use this part of the document to provide their name, date of birth, national security number, home address (if it is different from the child's residential address), contact details, and their relationship to the child. According to the instructions, the DLA for the child can be claimed by those individuals who are taking care of the child (parents, guardians, siblings, etc.)
Where to Send My DLA Form?
Generally, individuals will receive a DLA Application Form by calling the DLA helpline and requesting it in the mail. The application is sent with an enclosed envelope that already has a return address and stamp on it.
