How to Fill Out Form DS01?

Even though Form DS01 only consists of three pages, applicants must pay attention when filling it out and should not miss any parts of it. The form includes several parts, namely:

1. "Company Details." In the first part of the document, the applicant must designate the company's name and number. They must match the information held in the Companies House Register.
2. "The Application." Here the applicant can find information about the consequences of providing false information. In addition to this, this part includes a declaration from the directors of the company that they are acting in accordance with the law and certain provisions of the Companies Act 2006.
3. "Names and Signatures of Directors." This section of the form provides a space for directors of the company to state their names, signatures, and the dates when they signed it. This information will also be shown in the public record and it includes the individual's name, address, and telephone number.
