How to Fill in Sure Start Maternity Grant Form?

The applicant is supposed to provide the Department for Work and Pensions (DWP) with the following information:

Where Do I Send My Sure Start Maternity Grant Form?

According to the official website of the government of the United Kingdom, after the applicant has completed their Form SF100 and received a statement from a doctor or a midwife (if applicable), they are supposed to submit the claim to the DWP. They can do it by taking the form to the nearest Jobcentre Plus office or by posting it to the DWP's address (Freepost DWP SSMG). Applicants do not need to use a postcode or a stamp when posting the form to the DWP by using the Freepost address.
