What Is COSHH Assessment Form?

A COSHH Assessment Form (HSE) is a document that is supposed to be used when companies work with hazardous chemicals. The purpose of the form is to address the risk of possible harm to health and to describe methods that can be used to prevent exposure to the substances.

This form must be completed and signed before the work with hazardous chemicals starts. The COSHH Assessment Form was issued by the United Kingdom Health and Safety Executive (HSE) with the latest version available for download through the link below.

This form is valid for one year after it has been completed by the assessor. A COSHH Risk Assessment Form must be reviewed annually, and every time something has drastically changed after the last assessment form has been prepared.

How to Fill Out COSHH Assessment Form?

If a company uses or produces toxic chemicals, they are required to complete the assessment. To do so, they must obtain a blank COSHH Assessment Form and complete it. This document contains several parts, that the assessor must fill out. These parts include:

  1. In the first part of the document ("Introduction"), the assessor must designate their name and the date when the assessment has taken place. Additionally, they are required to provide the name of their employer (or the name of their supervisor) and the dates when the assessment has been reviewed.
  2. In "Hazards Identified," the assessor is required to provide the name of the substance that is being used or created by the company, describe its toxic properties, and indicate the amount of this chemical that is used. In addition to this, the assessor must designate what is the purpose of the chemical, how it will be used, and who is allowed to work with it.
  3. Assessors are supposed to use the next section of the form ("Methods of Prevention or Control of Exposure") to state how the company will avoid or control the exposure of the toxic chemicals. It can include information about the required engineering controls, access control, disposal procedures, storage requirements, and other details.
  4. At the end of the document, the employer (or supervisor) confirms that they acknowledged the content of this document and signs the document. The form must also be signed by the employee who undertakes the risk assessment work.
