How to Fill Out DA Form 5811?

This is why every accidental or intentional loss or damage caused to the ammunition must be recorded. Follow these DA Form 5811 instructions to outline what happened to the items and substances that were lost or damaged:

  1. Enter the stock number of the ammunition item, briefly describe it, and specify the number of items that were damaged or misplaced. Check the boxes to verify whether the items were lost or damaged and whether the negligence of personnel is the reason behind this.

  2. Use the blank space to list the events that led to the loss of ammunition or damage inflicted upon it. Once the first section of the form is completed, sign and date the paperwork.

  3. Evaluate the situation at hand - it is crucial to learn what led to the loss or damage of ammunition. You have to certify the events were analyzed and confirm whether the events occurred due to personnel's negligence, misconduct, or use of ammunition without proper authorization.
