1. Boxes 1 and 2 identify the prisoner by full name and registration number.
2. Box 3 labels the form as an admission summary or progress summary. This choice will affect the fields that will need to be filed.
3. Complete Line 5, Prior Offense, Line 6, Evaluation and Planning, and Line 7, Assignment Program, for admission summaries.
4. Complete Line 7, Assignment Program, Line 8, Confinement progress, and Line 9.

DD 2715-2 Related Forms

The form is used for gathering information related to the prisoner and may become part of the clemency and parole-related paperwork package.

• DD Form 2715, Clemency/Parole Submission, and DD Form 2715-1, Disposition Board Recommendation, are submitted along with the DD 2715-2 part of the parole and clemency-related forms package given to the Clemency and Parole Board for review;
