Form TR-212a Instructions

Provide the following details in the TR-212a Form:

  1. Indicate your county of residence, type of the place, and the date of application.
  2. State your full name and address. Record your driver's license number, federal employer identification number, taxpayer identification number, or social security number.
  3. Describe the vehicle - enter its identification number, fuel type, year, make, model, empty weight and gross weight, truck class. Record the date when the vehicle was purchased or brought to Kansas. State the actual mileage reading or certify the odometer discrepancy.
  4. Add the mailing address for the title, license plate, and registration.
  5. If the lienholder receives the title and registration, write down their name and address.
  6. If the vehicle was transferred on death, name the beneficiary who gets the vehicle.
  7. Provide the details of the plate transfer - description of the previous vehicle and name of the vehicle purchaser.
  8. If you add a name to the title or registration, this individual needs to sign the application and indicate their relationship to the existing owner.
  9. List the fees. In addition to the Kansas Vehicle Registration fees, you may have to pay DMV modernization fee, county service fee, sales tax, etc. Ask the DOV employee about the fees you need to pay.
