The main form - CBP Form 7512 - contains the information about the importer, the port of entry and unlading, the bond description, the reason for the transfer, and the record of the cartage or the lighterage along with the additional description of the goods. It is the main document that is filled out to sufficiently describe the cargo to identify it for the CBP.

  3. Make sure the kind of entry is clearly shown in the block in the upper right-hand corner on the first page of the form, whether it is used as an entry or withdrawal.
  4. Verify that the merchandise has been delivered in good condition, as noted on the original lading.
  5. Specify the entry number and the class of entry.
  6. Write down the marks and numbers of packages, describe them fully, as per shipping papers, including gross weight in pounds, value, rate, and duty.

The private parties may print the form if the printed version conforms to the official version in wording arrangement, size, color, and quality of the paper. The inspector's report includes the date of delivery to the importer or the general order, the description of the packages, the number and the kind of entry or the general order, the bonded truck or the lighter number, the conditions, the inspector's name, and signature.
