What Is Form TC-656?

Form TC-656, Vehicle Application for Utah Title, is a legal document used by Utah vehicle owners to apply for a title, change information on an existing title, or request a permit to dismantle a vehicle. Without a title, the purchaser of the vehicle will not be established as the legal operator of the vehicle and will not have proof of ownership. To get a title and registration for a vehicle you purchased, you will need this application, the original title or a bill of sale signed by the seller. Who Must File Utah TC-656?

At the top of the form, you need to indicate the reason for applying. Complete Form TC-656 in the following cases:

  • Applying for a new Utah title;
  • Changing the owner of the vehicle;
  • Changing the vehicle lienholder;
  • Correcting the title;
  • Obtaining a salvage title or non-repairable title;
  • Applying for a dismantling permit.

Provide the following details in the Vehicle Application for Utah Title:

  1. New owner information - driver's license number and state of issuance, full name, date of birth, addresses. Instead of a driver's license number, state a Federal Employer Identification Number (FEIN) if the owner is a business. You may add one more owner to the application who also needs to sign the document. Indicate the relationship of the owner to the co-owner;
  2. New lessee information - provide the same details and add a co-lessee if applicable;
  3. Description of the vehicle. Record its type, year, make, model, vehicle identification number, fuel type, color, body type. Enter the number of its cylinders and state the purchase price. Write down the current odometer reading;
  4. Type of the license plate and physical address of the vehicle;
  5. Lienholder information - name, driver's license number or FEIN, branch number, mailing address;
  6. Owner's signature and actual date;
  7. Purchase and dealer information if the vehicle was sold by a dealership - purchase date, dealer number, permit number, signature, and date.
