Physical Form Types

  • A Physical Examination Form is a generic template you may use whenever a patient asks you to create a detailed report regarding their physical status - the individual that scheduled an appointment may worry about painful sensations, regularly visit doctors due to genetic predispositions to illnesses, or require a formal confirmation of their excellent health and fitness prior to travel or permission to do certain activities;
  • Pre Employment Physical Form. Certain employers ask job candidates to undergo a health check before they are permitted to join the organization and work safely - police officers, healthcare workers, and firefighters must be examined so that it is confirmed they are able to perform their duties. The doctor will evaluate the person's physical health recording their conclusions and suggestions in this form - in some cases, they will also enter the results of a drug screening;
  • Sports Physical Form. Use this instrument to write down your observations during an examination of an athlete - or an individual that aspires to be one. Whether the person that booked an appointment with you has trials coming up and wants to get selected in a team of their choice or they need to participate in a local sporting competition, it is up to a doctor to certify their physical condition is at a proper level.

How to Fill Out a Physical Form?

Here are several tips you may follow when completing a basic Physical Form:

  1. Make sure you communicate with the patient (or their parent, guardian, or legal representative if you are examining a child or a person who lacks mental capacity) as you are checking their physical state. Their answers will help you see the whole picture and make appropriate judgements or draw up recommendations suitable for their situation - besides, many questions the form contains can only be answered by the individual in front of you.
  2. Opt for a questionnaire whether you are inquiring about a patient's medical history or assessing their symptoms at a particular moment. Simple "yes" or "no" questions will save you time - only ask for clarifications if the answer is out of the ordinary. As you are addressing the patient's concerns related to the existing health issue, try to connect them with the previous complications they have had or the medical history of their family members.
  3. Check the patient's blood pressure, pulse, and breaths per minute. Look at their general appearance - skin, nails, and hair - to notice any unusual signs of poor health. Record your observations and move towards a more thorough examination - check the patient's vision, hearing, nose, throat, and neck paying close attention to lymph nodes and glands. Listen to their heart and lungs, palpate the abdomen, and assess muscle strength in the patient's limbs. In case the patient complains about discomfort or worries about symptoms that are new to them, offer a pelvic exam or do a rectal exam. All your findings should be put in writing and verified by the patient.
  4. Provide clearance for the upcoming employment or another activity the patient is interested in or determine the person cannot be allowed to carry out the job in question or their health does not permit them to play sports or travel. You may offer recommendations to the patient in case you know what treatment will work and possibly delay your clearance until the symptoms are gone or the illness is dealt with. Sign and date the form and obtain the patient's signature as well. Keep a copy of the document for yourself and give another to the patient.
