What Is a Hurt Feelings Report?

DA Form IMT WF1, Hurt Feelings Report , is a form issued by the U.S. Department of the Army (DA). It was originally released on April 1, 1989, along with Field Manual 22-102 "Wall-to-Wall Counseling" (FM 22-102) as a joke.

The latest version of the report was released on April 1, 2009 , according to which the edition of April 1989 is now "obselete," intentionally spelled incorrectly. A fillable version of the Hurt Feelings Report is available for download below.

How to Fill Out a Hurt Feelings Report?

A Hurt Feelings Report consists of several parts. It starts with a title and a reference to Field Manual 22-102, which should be used along with this report. The next section is Data Required by the Privacy Act of 1974. This part of DA Form IMT WF1 consists of several types of data:

  1. The authority, which authorizes the solicitation of this information.
  2. Principal purpose.
  3. Routine uses (external sharing of information outside the agency).
  4. Disclosure. The form states voluntary disclosure of any information you provide.

The purpose of the form is to assist "whiners" in documenting their hurt feelings and to provide leaders with a list of soldiers who require additional counseling, non-commissioned officer (NCO) leadership, and extra duty.

The next part of the Hurt Feelings Report covers "Administrative Data." This is the part of the report dedicated to the personal information of the filer. This includes:

  1. Name.
  2. Rank/Grade.
  3. Organization.
  4. Social Security Number.
  5. Date of the report.
  6. Name and title of the person filling out this form.

Following is the "Incident Report," where the filer describes the incident by filling out specific charts. These charts cover a range of metrics, such as:

  1. Date feelings were hurt.
  2. Time of hurtfulness.
  3. Location of hurtful incident.
  4. NCO or officer sympathetic to the whiner.
  5. Name of the man or woman who hurt your sensitive feelings.
  6. Rank and grade of the person stated above.
  7. Organization of the person stated above.

In the "Injury" section the filer is supposed to mark the charts that are applicable for them. In the "Reason for Filing This Report" the filer is given 15 reasons for filing this report, from which they are supposed to mark all that apply. The chart offers different options, such as:

  • "I am a crybaby;"
  • "My feelings are easily hurt;"
  • "The army needs to fix my problems;"
  • "I was told that I am not a hero;"
  • "I was not offered a post brief;"
  • "My hands should be in my pockets."

In the "Narrative" part of the Hurt Feelings Report, the filer is offered to tell in their own words how their feelings were hurt. The last part of the Hurt Feelings Report is "Authentication." This segment authenticates the names and signatures of those who were involved in the incident: "real man/woman" and "whiner." At the end of the form, the U.S. Army claims to take hurt feelings seriously and promptly dispatch a "hugger" to anybody that needs one, as well as do other things to make the filer feel better.

The Army Hurt Feelings Report and Field Manual 22-102 "Wall-to-Wall Counseling" was released to commemorate April Fool's Day in 1989. The intentional use of sarcasm and spelling mistakes in the form doesn't have any other purpose except for humor.
