What Is Form DS-157?
Form DS-157, Supplemental SIV Chief of Mission Application, is a legal document completed by Afghan nationals employed in Afghanistan for two years or more on behalf of or by the U.S. government to apply for lawful permanent residence in the U.S. You can qualify for the Special Immigrant Visa (SIV) program if you meet these requirements:
- You are an Afghanistan national.
- You worked in Afghanistan for at least two years between 2001 and 2020.
- You were employed as a translator or interpreter for the U.S. government, performed various sensitive activities for the U.S. government, or the International Security Assistance Force, providing valuable and faithful service.
- You have experienced a serious threat as a result of your employment.

Form DS-157 Instructions
Follow these steps to fill out the DS-157 Form:
- Provide your personal details (your full name, name of the tribe or clan, other names and aliases you used, date and place of birth, passport number, national identity number, and sex). Identify your spouse, father, and mother. Enter your email address and phone number.
- Name any children you have that are currently under the age of 21 and write down their dates of birth.
- List the countries you have been to in the last ten years and countries that issued you a passport. If you ever lost a passport or had one stolen, check the appropriate box.
- Indicate your last five employers (their names and business addresses). State your job titles, supervisors' names and email addresses, dates of employment, and the reason for separation.
- If you have ever applied for the Chief of Mission approval, enter the application case number.
- Sign and date the form.
