What Is TRADOC Form 350-18-2-R-E?

TRADOC Form 350-18-2-R-E, Unit Pre-execution Checklist , is a formal document prepared by the military unit commander to confirm the soldier that is going to be enrolled in the institutional training meets the course requirements. Pre-Execution Checklist Instructions

Follow these steps to complete an Army Pre-execution Checklist:

  1. State the name of the soldier and their unit. Indicate the title of the course and the date of the report.
  2. Confirm the soldier is prepared to perform training in accordance with the army standards. You must ensure that all planning and training have been conducted properly and then sign the form. Both soldier and supervisor must write their initials alongside each step of preparation completed - show the soldier has received all required information, clothing, and equipment, they meet physical standards and have enough funds for travel, they have been medically evaluated and assessed for special skills. Certify their transportation was verified, they have an identification card and identification tags. Attach documentation to confirm your statements - for instance, if the future student must attend the prerequisite course, enclose a copy of the Form DA 1059 to verify their successful completion of the course.
  3. List telephone numbers of all points of contact of the unit - it is necessary to provide the contact details of the unit commander, first sergeant, and field training manager. Add the email addresses of the first sergeant and the command sergeant major.
  4. Provide the minimum aptitude score - show whether the soldier can excel in multiple occupational specialties.
  5. Assess the physical characteristics of the soldier and write down their scores.
  6. If the soldier is a military or civilian vehicle operator, list their license numbers, expiration dates, and the state of issuance.
  7. If needed, attach a security clearance, Form DA 3349, and a statement that confirms the results of the medical retention board. Use free space to list other requirements and waivers if necessary.
  8. Obtain the student's signature that certifies they have been fully informed about the course they are about to attend.
  9. Confirm you have reviewed the qualifications and skills of the soldier and you are sure they are ready to attend and successfully complete the course. Write down your name and sign the form.

Send the documented evidence of the soldier's preparedness to take the course to the course administrator before the scheduled classes start.
