COMMISSIONER
1.
Step 1—October 1—Complete “Procedure for Focused Selection Verification.”
2.
Step 2—October 1—Someone other than the determining official must confirm the
determinations for each of the applications selected for approval are correct. Make sure
they sign and date the applications in the Confirming Official’s area.
3.
Step 3—October 15—Complete Phase 1 and 2 on the “Verification Summary Report” on
the CNP system under “Verification.”
4.
Step 4—October 1—Complete the first three columns of the “Application Verification
Summary” worksheet. (THIS IS AN OPTIONAL FORM)
5.
Step 5—Send out “We must Check Your Application” letters. Keep copies or notate the
date you sent the letter.
6.
Step 6—If Households do not respond, remember you must make at least one
documented attempt to obtain the necessary information by mail, by telephone, by e-mail,
or through personal contact.
7.
Step 7—After you document the one follow up attempt (Step 6), sign the application and
date it where it indicates Follow-Up Official’s Signature.
8.
Step 8—After you receive income documentation from the household, and make the
eligibility determination, send out the “We Have Checked Your Application” letters.
9.
Step 9—November 30 Complete Phase 3 on the “Verification Summary Report” on the
CNP system under “Verification.”
10.
Step 10—Complete the fourth column of the “Application Verification Summary” worksheet.

11.
Step 11—February 15—Complete Phase 4 on the “Verification Summary Report” on the
CNP system under “Verification. (THIS IS OPTIONAL.)
12.
Step 12—February 15—Complete the fifth column of the “Application Verification
Summary” worksheet. (THIS IS OPTIONAL.)
13.
Step 13—February 16—Submit the completed “Verification Summary Report” on the CNP
system.
