I have read the referenced Regulation of Connecticut State Agencies pertaining to the notices required regarding franchise(s) and/or manufacturer(s) insolvency,
dissolution, or termination of business. I will provide this notice by certified mail to the Department of Motor Vehicles, Dealer and Repairer Division, 60 State Street,
Wethersfield, CT 06161 within the required time period.

Notice Required from Manufacturer upon Termination of Franchise
Manufacturer Name:
Section 14-67a-4 of the Regulation of Connecticut State Agencies. Application for
manufacturer's license
(a) In addition to any other notice requirements contained in chapter 739 of the general
statutes, a manufacturer as defined in section 14-1 of the general statutes, upon making
application for a license to engage in the business of manufacturing motor vehicles for sale in

this state as provided in section 14-67a of the general statutes, shall consent to file with the
commissioner of motor vehicles a notice as provided in subsection (b).
(b) Each licensed manufacturer shall notify the commissioner, in writing, on a form or in a
format as prescribed by the commissioner, within three (3) business days after first obtaining
knowledge with respect to any of the following:
(1) Its intent not to renew a franchise;

(2) The institution of an action to cancel or terminate a franchisee or distributor of the
manufacturer;
(3) The abandonment of a franchise by a franchisee or distributor of the manufacturer; or
(4) The filing of a petition in bankruptcy, or for dissolution or other termination of business, of a

Such notice shall include, if known, the dates of any such actions or impending actions, and
any other relevant information required by the commissioner.
Manufacturer License #:
(c) Such notification shall be made in writing by a person having authority to act for the
manufacturer.
