This form is required for compliance with Construction General Permit, Order No. 2009-0009-DWA (as amended by 2010-0014-DWQ and
2012-006-DWQ).
This form is to be completed when the contract has a bid item for Permanent Erosion Control Establishment.
Include a copy of the completed form in the project Storm Water Pollution Prevention Plan (SWPPP) files.
