INSTRUCTIONS (OIL WELL POTENTIAL REPORT)
1.
THIS FORM SHALL BE FILED IN DUPLICATE WITH THE DISTRICT MANAGER OF THE
CONSERVATION DISTRICT IN WHICH THE FIELD IS LOCATED
2.
DM-1-R’S MUST FILED ON OR BEFORE MAY 1
AND NOVEMBER 1
EACH YEAR.

FAILURE TO FILE BY THESE DEADLINES WILL RESULT IN CANCELATION OF YOUR
COMPANY’S ALLOWABLES.
3.
WELLS ARE TO BE LISTED IN ASCENDING LUW CODE ORDER AND WITHIN EACH LUW
CODE IN ASCENDING SERIAL NUMBER ORDER.
4.
NO TEST SHALL BE COMMENCED UNTIL THE WELL FLOW HAS STABILIZED, AND THE
WELL SHALL REMAIN ON A CONSTANT CHOKE THROUGHOUT THE TEST PERIOD.
DURATION OF THE TEST SHALL BE OF SUCH LENGTH AS TO DETERMINE AN ACCURATE
GAUGE.
5.
ALL TEST DATES SHALL BE WITHIN SIX (6) WEEKS PRIOR TO THE DUE DATE OF THIS
REPORT. (I.E. FOR REPORTS DUE MAY 1, THE TEST DATE CANNOT BE PRIOR TO MARCH 15)
6.
FORMS MUST BE TYPED USING A FONT SIZE THAT IS LEGIBLE AND ALLOWS
REPRODUCTION.
7.
INSTRUCTIONS FOR SPECIFIC COLUMNS:
A. PERFORATIONS MUST MATCH WHAT IS CURRENTLY ON FILE IN SONRIS.
B. GAS-OIL-RATIO (G0R) FOR GAS LIFT WELLS WILL BE CALCULATED USING NET
FORMATION GAS.
C. SHUT-IN PRESSURE OF THE WELL WILL BE REPORTED IF TAKEN SINCE THE LAST DM-
1-R WAS FILED FOR THE WELL.
D. INDICATE PRODUCING METHOD (PM) BY USING ‘F’ FOR FLOWING, ‘P’ FOR PUMPING
OR ‘G’ FOR GAS LIFT.
E. INACTIVE WELLS MUST BE REPORTED USING ONE OF THE FOLLOWING STATUS CODES
UNTIL SUCH TIME AS THE WELL IS PLUGGED AND ABANDONED:
INACTIVE - FUTURE UTILITY - 1
INACTIVE - NO FUTURE UTILITY – 2
F. THE STATUS DATE FOR INACTIVE WELLS SHOULD BE THE ACTUAL SHUT-IN DATE
