Instructions for BLR 09210
This form shall be used to indicate the annual program funds projected to be requested from the Township Bridge Program (TBP) for Road Districts. It shall be submitted to the district
office on or before September 1 each year. A location map showing the location of all structures listed must also be submitted with this form. For more information, refer to the Bureau
of Local Roads and Streets Manual (BLRS), Chapter 9.
County
Insert the name of the County in which the Road Districts are located.
Program for Fiscal Year
Insert the State Fiscal Year for the program.
Prior Fiscal Year(s) Balance
Insert the total available balance of all prior fiscal years.
Current Fiscal Year Balance
Insert the total of the current fiscal year.
Total Available TBP Fund
This is a calculated field, no entry is necessary. This is the total of the prior fiscal year(s) balance plus the current fiscal year balance.
Priority
Insert the priority rating for each project listed by number, with 1 being the highest priority.
Road District
Insert the name of the Road District the project is for.
Section Number
Insert the section number assigned to this project.
TR Number
Insert the Township Route (TR) number on which the project is located.
Road Name
Insert the name of the roadway on which the project is located.
Existing Structure #
Insert the existing structure number, is applicable.
Feature Crossed
Insert the feature crossed by the structure.
Total Cost
Insert the total estimated cost for each applicable work type.
TBP Cost
Insert the estimated TBP cost for each applicable work type. This cannot exceed 80% of the total cost.
Anticipated Letting Date
Insert the anticipated letting date for each project, if applicable.
Total Cost
This is a calculated field, no entry is necessary. This calculates the total estimated cost for each project and the total estimated TBP cost.
Total Program Cost
This is a calculated field, no entry is necessary. This calculates the total estimated program cost.
Location Map
Attach a location map showing the structure location, along with the proximity to the nearest town or municipality.
One original must be submitted to the Regional Engineer's District office.
