SURFACE MINING OPERATIONS IN THE STATE OF IOWA.
Know all men by these presents,
that

as surety are firmly bound unto the State of Iowa, Division of Soil Conservation Water Quality, the penal sum of $
lawful

executors, administrators, or successors, and assigns, jointly and severally, firmly by these presents.
The condition
of this obligation is that whereas the above bounded principal proposes to conduct mining operations in and upon
land situated within the State of Iowa, and more particularly described in the attachment to this bond (NOTE: This form shall be
accompanied by an attachment which lists, by name and location, all sites to be operated under this bond), said land being registered
pursuant to Iowa Code Chapter 208 and including such registered lands and adjoining lands affected by the mining activities of the
principal or his agents.
Now, therefore,
if the above bounden principal shall fully comply with all of the provisions of Iowa Code Chapter 208 and the rules of
the Division of Soil Conservation and Water Quality with respect to the reclamation and conservation of the lands affected by mining
operations of the principal or the principal’s agents and other lands registered pursuant to Chapter 208, then this obligation shall be void,
otherwise the same shall remain in full force and effect on and after

released and shall continue in full force and effect with respect to lands which have become “affected lands” under the provisions of

General Instructions
Unless instructed to the contrary by the Division of Soil Conservation and Water Quality or an
official representative of the Division Director, the minimum allowable bond shall be the greater of
$2,000 per site or $500 per acre affected by the mining process.
The home office address of the Surety shall be included, in full, so that all correspondence and
terminated bonds may be forwarded to the home office for appropriate response. The full office
address of the resident surety agent shall also be included. All blanks shall be filled out completely
and accurately, and type written unless otherwise requested.
An attachment, as noted in paragraph three of the reverse text, must be included to validate this
surety bond. The attachment must list the bond number, the sites to be covered by the bond and their
locations, and be counter signed by representatives of both the bonding company and the principal
(see sample below).
A certified copy of the Surety’s Power of Attorney for the individual executing the bond on behalf of
the company shall be attached to the original bond as well as any future addenda, riders, or
attachments.
