What Is Form SSA-7050-F4?

Form SSA-7050-F4, Request for Social Security Earnings Information is a form used for requesting detailed information about your earnings that affect your Social Security benefits and retirement amount. This information is provided by the Social Security Administration (SSA). The latest version of the form - also known as the SSA Form 7050-F4 - was issued in March 2019. All previous editions of the document are obsolete. Form SSA-7008, Request for Correction of Earnings Record is a related form used by the SSA to correct earnings records when a discrepancy appears.

The SSA uses the information you provide in this form to identify proper records and to send you the requested information. Completion of the document is voluntarily, but if you do not furnish all the necessary information, the SSA will not be able to process your request. One form is applicable for one Social Security number (SSN) only. The SSA charges a $91 fee to provide information unrelated to the administration of the programs. They may certify the itemized earnings information and the yearly totals of earnings for an additional $34 fee.

You may obtain the information about someone else's earnings if you are a natural, adoptive parent or a legal guardian of this person. You also have a right to request earnings information from the file of a deceased person if you are a surviving spouse, parent or child, legal representative of the estate, or individual with a material interest. In this case, you need to submit documents that prove your relationship.

voluntary; however, failure to do so may prevent your request from being processed.

Social Security benefits. However, we may use it for the administration and integrity of Social Security programs. We may
also disclose information to another person or to another agency in accordance with approved routine uses, which

another means of payment (for example, by check or money order). If you choose the credit card payment option, we will
provide the information you give us to the banks handling your credit card account and the Social Security
Administration's (SSA) account.

How to Fill out Form SSA-7050-F4?

To get your earnings statement, fill out the Social Security Form SSA-7050-F4 as follows:

  1. Provide your first name, last name, and middle initial. It should match the information on your Social Security card.
  2. Specify your SSN.
  3. Provide your date of birth.

What Supporting Documents Do I Need with Form SSA-7050-F4?

When submitting the Request for Social Security Earnings Information, you need to attach some of the following supporting documents:

  • A check or money order that shows that you paid the requested fee;
  • If you want to pay the fee with a credit card, complete the credit card section and attach it to the request form;
  • If you act on the behalf of the minor child or incompetent individual, attach the proof of your relationship (use Form SSA-7050-F4). You can include an adoption decree, birth certificate, court order, or any other legally binding documents;
  • If you request earning information of a deceased person, include the proof of death and the document that testifies your relationship to the deceased person.

You have to send all the required documents at the same time, in order to avoid confusion and rejection of your request.
