What Is Form DS-64?

Form DS-64, Statement Regarding a Lost or Stolen Passport, is a document issued by the U.S. Department of State (DS) for individuals who want to report that their U.S. passport was stolen or lost. After submitting the form, an individual's passport will be invalidated to avoid identity theft. If the passport is found later, it may not be used.

If an applicant wants to report a lost or stolen U.S. passport and replace it, they should submit Form DS-64 with Form DS-11, Application for a U.S. Passport, in-person to any designated acceptance facility, U.S. consulate abroad, U.S. embassy, or U.S. passport agency. To make an appointment (if required) an applicant can call the National Passport Information Center or use the online passport appointment system. Even though the DS-11 is expired, it is still in use until the U.S. Department of State issues an updated version of the form.

Download a copy of Form DS-64 via the link above. Here's how you can submit your application to the U.S. Department of State:

  1. Apply Online. To report a lost or stolen passport, visit the official website of the U.S. Department of State and submit your DS-64 Form online.
  2. Apply by Mail. Mail a completed copy of Form DS-64 to the U.S. Department of State at the following address: CLASP, U.S. Department of State, CA/PPT/S/L/LE, 44132 Mercure Cir, PO Box 1227, Sterling, VA 20166-1227. In this case, you'll be required to submit the form with a photocopy of government-issued photo identification (for example, a driver's license).
  3. Apply by Phone. As per the DS-64 instructions, you can report the loss of the document by calling the National Passport Information Center and following the representative's instructions.

Form DS-64 Instructions

  1. Select the document(s) that you are reporting (either the U.S. passport book or U.S. passport card) and if it was lost or stolen.
  2. In the first part of the application ("Identifying Information of Passport Holder.), you will be required to provide identifying information, including your full name, date of birth, place of birth, sex, social security number, current address, telephone number, and email.
  3. The second part of the document ("Lost or Stolen U.S. Passport Book/Card Information") is used to provide data about the document and the circumstances of how it was lost or stolen. Explain how their passport was stolen, provide details on where and when it happened (including address), list the information about the documents that were lost or stolen (number and date of issuing). Specify if you're applying for a new passport in connection with this application.
  4. The last part of the application ("Signature And Date") will require you to specify that you've read and understood the warning on page one of this form, that the information provided on the form is correct and complete, and that the U.S. passport book (or U.S. passport card) was not voluntarily given to another person or disposed of it in an unauthorized manner.

The passport that has been reported as missing will be invalidated and may not be used again. If the original passport is found or recovered, it must be immediately returned to Passport Services or the nearest U.S. passport agency, U.S. embassy, or U.S. consulate, if abroad.
