Indiana Vehicle Inspection Form Instructions

Here is how the approved inspectors must complete the State Form 39530:

  1. Provide the owner information - their full name and address.
  2. Describe the vehicle or watercraft in question. Indicate its identification number or, if no identification number was found during the inspection, tick the box "none". The main purpose of this inspection is to make sure the identification number does not belong to a stolen car or boat. Record the make, model, year, type, plate number, and the state of the previous registration. If you are inspecting a watercraft and it has a registration number, write it down.
  3. If you were asked to perform a visual inspection of the assembled car or vessel, you need to enter the serial numbers for component parts present: engine or motor, body chassis, rear clip, transmission, front assembly, frame. If there are other major parts, specify them and list their serial numbers in the available space.
  4. State when you performed a check and add any comments if necessary. Only fill out this part if you are a police officer and you will submit the inspection details in the Indiana Data and Communications System or the National Crime Information Center.
  5. Confirm all statements in the form are true to the best of your knowledge. Date and sign the form. Write down your contact information (address, telephone number, and e-mail address). Enter your full name and title. If you represent a police department, dealership, or branch, indicate your badge, dealer, or branch number respectively.

Once this form is prepared, the vehicle or boat owner will be asked to pay a fee of not more than $5. Then, the owner will be able to bring the form to the BMV office and obtain a title and registration for their item.
