Form MV-351 Instructions

In order to complete this form, a motor dealer or a full agent will need to provide important information which includes:

  • The name of the business as it appears on the registration card, dealer ID or agent services ID number (the five-digit number beginning with 83-XXX, 85-XXX, 86-XXX, and so on);
  • Business address as it appears on the registration card;
  • Business phone number.

Registration plates can be ordered in multiples of ten, with some exceptions - such as the Implement of Special Mobile Equipment, school bus, motor home, bus, and omnibus. Any of these can be ordered in multiples of five. For each class of registration plates requested, a separate Form MV-351 must be used with a separate check attached for each document. The amount of check or money order accompanying Form MV-351 must be stated, and if applicable, the name of the person authorized to pick up the order of registration plates must be indicated in the appropriate box. An application must be signed by the applicant.

In certain situations - if one or more plates or a rear window decal is missing from the ordered supply of temporary plates and decals due to loss, theft, or damage - use Form MV-351A, Application for Replacement of Temporary Registration Plate.

To complete this form, enter a dealer services identification number (DIN), name of the business, full address, and a business phone number. The registration plate number to be replaced must be entered with the list of WID numbers from receipt for missing registration plates. An application must be signed and dated, with the reason for using Form MV-351A provided.
