I agree to call the nearest Division of Plant Industry office within a 24-hour period after arrival at the above
address to set up an appointment to have the plants inspected. I also understand these plants are
allowed entry into the State of Florida and may be subject to a post-entry quarantine. The plants are not
to be moved from the above address until they are inspected by an authorized inspector of the Division
of Plant Industry.
Uncertified non-commercial shipments of firewood entering the state for personal consumption will be
allowed to enter under the condition that the firewood will be consumed, burned or destroyed as soon as
possible and not transported beyond the destination listed above.
The Florida Department of Agriculture and Consumer Services, Division of Plant Industry, retains the
right to destroy plants found infested with injurious plant pests that are detrimental to Florida agriculture.
Persons failing to comply are in violation of Florida Statutes 581.031(4) and Rule 5B-65.005(2), F.A.C.,
and may be subject to legal action.
