REPORTING FORM FOR DEFENSE-RELATED DISPOSITION
STATE OF ARKANSAS
CIRCUIT COURT: CRIMINAL
Instructions
The prosecuting attorney shall complete this disposition information. If there was a commitment to the
Arkansas Department of Correction, probation, suspended imposition of sentence, commitment to
Arkansas Community Correction or a county jail, a fine, restitution, or court costs on any charge then a
Sentencing Order should be completed instead of this form.
1. Complete the docket number, case styling (the full name of the defendant), and the defendant’s
driver’s license or state ID number.
2. Complete the date the information was filed, the arrest tracking number (ATN) and the arrest date.
If no arrest was made, write “N/A”.
3. Indicate the trial type. Indicate jury trial if a jury was sworn, regardless of whether a verdict was
reached. Indicate bench trial if evidence was introduced, regardless of whether a judgment was
reached.
4. If this case was a petition for post-conviction appeal or an extraordinary writ, please indicate
whether the petition was granted or denied and the case disposition (judgment, dismissed, etc.)
Skip the table of charges and go to the interpreter question.
5. If the case was consolidated into another case, complete the new case number. If all charges were
consolidated, skip the table of charges and go to the interpreter question.
6. Provide the manner of disposition for each charge. Add an additional sheet if needed.
a. Plea choices are no plea, not guilty, and not guilty by reason of mental disease or defect.
b. Charge disposition choices are acquitted, acquitted-mental defect, dismissed, dismissed-speedy
trial, and nolle prosequi.
7. Indicate whether an interpreter was used in the case. If so, indicate for whom and what language.
8. Indicate whether any party was self-represented (pro se) at any point in the case after the first
appearance.
9. Indicate if this person completed a specialty court program, such as drug court or veterans’ court,
that resulted in the dismissal of charges.
10. Complete the name and bar number of the attorney completing the form.
11. Include the judge’s signature, as required by Administrative Order 8.
