1. This Information Addendum is bound by the same approval and nullification requirements and stipulations as the
Application for Approved Commercial Production Suidae Herd Health Plan, FDACS-09153

2. This addendum provides additional information related to suidae animals and shall be attached to the Application for Approved Herd
Health Plan, FDACS-09153.
A.
All feral/wild suidae imported into a facility from another state (interstate movement) shall comply with current Florida Department
of Agriculture and Consumer Services (FDACS) Rules and Regulations.
B.
All feral/wild suidae that are brought into or shipped out of the facility from within the state (intrastate movement) shall comply with
the following requirements:
Prior to movement, the shipper shall obtain a movement permit from the Division of Animal Industry, Florida Department of
Agriculture and Consumer Services (FDACS) at (850) 410-0900.
This movement permit shall allow the feral/wild suidae to move to the facility listed above or directly to a USDA-approved
slaughter facility without individual animal identification or Brucellosis or Pseudorabies testing.
C.
Herd owner/operator shall maintain and provide to the Florida Department of Agriculture and Consumer Services (FDACS) for
inspection a record of movement for all feral/wild suidae moved into and out of the facility listed above.
D.
Upon request, the herd owner/operator shall provide to the Florida Department of Agriculture and Consumer Services (FDACS),
appropriate tissues or blood samples for diagnostic laboratory testing on feral/wild suidae which dies or are slaughtered by any
method at the facility listed above. Samples shall be obtained by trained personnel and immediately submitted to the State
Diagnostic Laboratory in Kissimmee, FL.
I hereby agree to the provisions stipulated in this ADDENDUM TO APPLICATION FOR APPROVED COMMERCIAL
PRODUCTION SUIDAE HERD HEALTH PLAN and understand that this does not negate any regulatory provision required
by other agencies involving their jurisdiction over any animal under my ownership or control.
