APPLICATION FOR ORDERING VANITY LICENSE PLATES
APPLICATION INSTRUCTIONS:
1.
If application is approved, a card will be returned to you when the vanity license plates are ready for pick up.
2.
When picking up vanity license plates, be sure to bring in the card mailed to you, along with current registration card, current
license plates, all required fees and a registration application form (TR-1), to conduct a plate change transaction. If the
registered owner will not be present, the registration application (TR-1) will need to be notarized.
Plates WILL NOT be held after ninety (90) days of notification.
